REGISTRATION FORMS (from 10/10/13 SU Meeting)

Girl or Adult Registration Forms (writable forms available online), 

• Check form for completeness:
• Name
• Address
• Phone
• Email (this ensures that parents will receive GS of CT notifications)
• At least one parent/guardian's name on Girl registration forms
• Parent's signature
• Troop Registrars should:
• fill out the bottom section: MMM=302, RPT CODE=601 and Troop #xxxxx
• verify payment received
• obtain forms/signatures on Financial Aid forms, if applicable.

Membership Dues Summary Report (writable forms available online), 

• MMM=302, RPT CODE=601 & REG AREA=003  
• The volunteer listed as "filling out the form" should always be your "O1" Leader. 
• For meetings information, always fill in the FULL NAME, ADDRESS and TIME/DAYS FOR THE MEETINGS AS THIS IS WHAT ENSURES PROPER INSURANCE COVERAGE FOR TROOP.  
• Membership Summary Report & all Registration Forms should be submitted to our AMITY Registrar (Mary Cassidy) IN DUPLICATE (and keep copies for your troop records also).   
• Please remember to blacken out credit card information on copies retained by the troop and ALWAYS safeguard the privacy of all forms.
